Menaḥem, and mentioning Mevorakh b. David. Verso: part of a poorly preserved power of attorney, from the court of, and probably written by, Efrayim b. Shemarya. Dated: Tishrei 1396, 7, or 8 = 1034–37 CE (probably 1398/1037 based on the dating of recto). Involving the wife of ʿAmram b. Saʿīd and two daughters of Aharon b. Saʿ[īd] ha-Levi. Witnesses include Yeshuʿa b. S[...], Menashshe b. Yehuda, and Yiṣḥaq ha-Kohen b. [...] among the witnesses. (Information from CUDL.)
