Brief text in Hebrew, document type or literary genre is unclear (colophon?). Almost none of the main text is preserved except the conclusion וכן יהי רצון. The location is then given as Damascus (חדרך סוריא), and the date is given as Tevet 1525 Seleucid is visible (טבת אתקכה) which is 1213/14 CE, and then possibly the word maʿāmīl (pl. of maʿmūl), significance unclear. The date may also be 1565 Seleucid = 1253/54 CE.
