Receipt for the capitation tax in Fustat for the Jewish oculist (al-yahūdī al-kaḥḥāl) Makārim b. Isḥāq, known from several other documents. Dated: 637 AH = 1239/40 CE. The total paid is 2 dinars. Mentions that the sum is "out of the that which was extracted by the government employee in Fustat" (mimmā ustukhrija ʿalā yad al-mustakhdam bi-miṣr). There are two ʿalāʾim partially preserved at the top.
